1. Collect the Documents

  • Proof of U.S. Citizenship: Valid U.S. birth or naturalization certificate or valid expired passport.
  • Proof of Identity: Valid driver’s license, military identification card or a current government or employer ID card.
  • Passport Photo: A passport-style photograph with a white or off-white background.
  • Fee:
    • $110 to $145.00 (depending on age and type of passport)
    • Separate check payable to U.S. Department of State.
